Health Clearance Forms and CPR cards are due August 3rd. ($50.00 late fee starting August 4th) Submit to Student Services Office. Health Clearance Forms must be in a sealed envelope with your name and contact number printed on the front. Check all pages of the form carefully prior to submittal (incomplete forms will not be cleared by the Occupational Health Nurse). |
